Please fill out the form at the bottom of this page. I set up appointments after I have your full name and employment information OR Date-check.com ID/reference. Being well-established on Date-check helps, but this does not neccesarily guarantee you an appointment with me if I feel the information is not sufficient, and I may request more information from you

If you’re not comfortable sharing your information as asked for in the form, I understand, but will not be able to schedule with you.

The reason I do this is for safety's sake - mine and yours. Safety and privacy are my very top concerns and any information I collect from you is for my own protection, and kept strictly confidential. After we meet I safeguard that all information we exchanged is completely deleted/erased/gone.

When I receive your form the first thing I will do once I read it is email you at the email address you provide (not your work email, unless that is the only one given). I will not check any of the information you provide me until I communicate with you about how I will do it first and make sure this fits in with your discretionary needs. I have been doing this for over a year and have never had an issue or caused an issue for anyone. I am extremely organized, professional and considerate about my screening process
